COVID-19 Vaccination Appointment Scheduling
Background
CSULB is providing the Moderna COVID-19 Vaccine to students, faculty, and staff at this time. Follow the steps below to get your vaccine:
Schedule an Appointment
To schedule an appointment, please click on any one of the days below. You do not have to arrive at your appointment at the specified time, but you must arrive between the hours of 9am and 4pm on the day of your appointment; otherwise your appointment will be cancelled and you'll have to make a new appointment. You'll receive a confirmation email from the vaccination appointment system, please save this email as you'll need it on the day of your vaccination.
On the Day of Your Appointment
On your vaccination appointment day, you are required to complete the COVID-19 Pre-screen questionnaire found in the Campus SingleSign-On (SSO) system. On your appointment date only, select Vaccine Clinic and complete the questionnaire. You can only visit campus if you're cleared to do so. You'll receive an email with your clearance to visit campus and the Vaccine Clinic. Please save the email as you'll need to show it at the Vaccine Clinic.
Be Prepared:
- Bring Your Appointment Confirmation (Email Printout or on your mobile screen)
- Bring Your COVID-19 Pre-Screen Confirmation (Email Printout or on your mobile screen)
- The vaccine is administered as an injection in the upper arm, so wearappropriate clothing.
