Open Enrollment

Open Enrollment is your opportunity to review & make changes to your medical, dental & vision plans. During Open Enrollment you can add, delete or change plans, or add/remove dependents from your coverage. To help navigate through Open Enrollment, we have included several frequently asked questions for your review.

Frequently Asked Questions

 

The Open Enrollment period will be held from Wednesday, November 2, 2016 through Friday, November 18, 2016. The deadline to submit changes to Human Resources is Friday, November 18, at 3:00 pm, and elections made during Open Enrollment will become effective January 1, 2017. 

Employer Contributions
The monthly contributions have increased to the amounts listed below, and the Forty-Niner Shops will contribute these amounts toward your total monthly premiums based on the coverage group you have selected from our plan options:

  • Employee Only - $607.78/month

  • Employee + One - $1,266.50/month

  • Employee + Family - $1,812.69/month

Medical:

  • Kaiser

  • Anthem Blue Cross HMO & HMO Select

  • Anthem Blue Cross PPO

Dental:

  • Delta PPO

  • MetLife/Safeguard HMO

Vision:

  • VSP Plan

Voluntary Life, Accidental Death & Dismemberment Plan:

The Shops offers a Voluntary Life and Accidental Death and Dismemberment Plan (AD&D), which will allow you to purchase additional Life and/or AD&D coverage for you and your family. These voluntary plans are offered through The Hartford and provide peace of mind that your family will be taken care of should something unexpected happen.

Flexible Spending Account (FSA) Plan:

The Flexible Spending Account (FSA) is a voluntary plan where you elect to have automatic deductions taken from your paycheck and put into a Flex Plan so you do not pay taxes on that amount. The money you elect to deduct can be used to reimburse you for medical expenses and/or for childcare expenses that you incur during the plan year, depending on the deduction amounts for each plan in which you enroll. This is a voluntary plan. Enrollment is not automatic and you must re-enroll each year to participate.

  • For medical expenses: You can get reimbursed for medical co-payments, prescriptions (excluding over-the-counter medications), glasses and dental procedures. A detailed list of qualified items is available on our website. The maximum deduction amount for 2017 is $2,600.

  • For childcare expenses: You can get reimbursed the same way. Based on your annual childcare expenses, you can estimate the amount and deduct it from your paycheck. You would then be reimbursed for your qualifying expenses. The maximum deduction amount for 2017 is $5,000. To learn more about these additional plans, please visit our website.

As you review your plan documents and wish to make changes, please contact Human Resources for the appropriate forms. The Open Enrollment Change Notification Form and all other forms must be submitted to Human Resources by Friday, November 18, at 3:00 pm.

If you are satisfied with your health coverage and do not want to make any changes, simply complete the Open Enrollment Change Notification Form to notify our office that you do not wish to make any change to your existing plan coverages for the 2017 plan year.

The Shops offers a Benefits Waiver Program. If you have medical, dental, and vision coverage outside of the Shops, the Program pays you cash in lieu of health insurance coverage. You must provide proof of coverage to participate and must provide every plan year to qualify. To learn more, please visit our website.

Please note that the Forty-Niner Shops’ monthly employer contributions are for Forty-Niner Shops sponsored health plans only, and do not apply to insurance programs outside of the Forty-Niner Shops.
