2019 -2020 Satisfactory Academic Progress (SAP) Forms

Summer Disqualification: If you received a SAP disqualification email and are enrolled in Summer 2019 term, you must make an appointment with a Beach Central Counselor by “Joining the Queue” to obtain the SAP Appeal form if you chose to appeal.

19-20 SAP Suspension Appeal

Students who fail to meet the GPA and/or Pace SAP Standards for two consecutive semesters are considered to be ineligible to receive financial aid. Students who wish to appeal their disqualification status may do so by submitting SAP Suspension Appeal form. An appeal must be based on extenuating circumstances that  impacted your academic performance.

19-20 SAP Maximum Time Frame Appeal

Students who have been disqualified from receiving financial aid due to exceeding the 150% of their program length may appeal their disqualification status by submitting the SAP Maximum Time Frame Appeal form.

19-20 SAP Disqualified After Probation Appeal 

Students who fail to meet all CSULB SAP Standards after one semester on Probation are ineligible to receive additional financial aid. Students who wish to appeal their disqualification status may do so by submitting the SAP Appeal – Disqualified After Probation Appeal form. Due to the severity of the SAP Standards deficiency, the included Academic Plan must be submitted along with the appeal form.

Concurrent/Visitor Enrollment Form  This application is to be used by California State University Students who wish to enroll concurrently or as a visitor at another CSU campus. Cross Enrollment Form (UC & Community College) This form is used by California residents currently enrolled at CSULB, who wish to enroll in one undergraduate course per academic term at a participating University of California campus or California Community College. Contact Information Change Request Current CSULB students can change their contact information through their MyCSULB Student Center.  This form should only be used by former students/alumni who need to update their contact information.    Contact Information Change Request Form (PDF)
Educational Leave Form  Any registered student, undergraduate or graduate, in good academic standing may request an Educational Leave. Students requesting an Educational Leave must complete an Educational Leave Form, in advance, including an explanation of their reasons for seeking the leave and a statement of when they intend to resume academic work.  An educational leave is a minimum of one semester or a maximum of two semesters.  For more information, please visit CSULB's Educational Leave.   Educational Leave Form (PDF)
Exception to Academic Policy for Undergraduate Limit This form is used in circumstances where you need to repeat a course more than once and/or repeat a course for audit purposes.  This is an exception to the academic policy on undergraduate limits. You may not enroll in or receive unit credit for a course for which you have already received a grade of A,B, C or CR, including a university-level course taken at a high school or another college or university.   Exception to Academic Policy for Undergraduate Limit Form (PDF)
Grade Option Form  This form is used for purposes of changing the grade option for a class you are currently enrolled in.  You have the option of auditing a class, changing the grade option from credit to no credit. In order to use this form, you must already be enrolled in the class.  This form is only valid for use until the last day to enroll in the term. If you are not enrolled in the class for which you are attempting to change the grade option, you must also submit a Late Registration form to add the class along with this form.   Visit Understanding Grades and Grading for more information.    Grade Option Form (PDF)
Late Registration  Form  Once self-service registration ends for the semester (after the 2nd week of classes), students must add courses in person at Enrollment Services (BH-101) using this form.    Late Registration Form (PDF)
Legal Name Change Request Form  In order for Enrollment Services to change your name or a name correction on legal documents, you must fill out this form along with the appropriate documentation requested. This form is only used by current and former students.    Legal Name Change Form (PDF)
Military Leave Form  A military leave is granted to students who serve in the Armed Forces of the United States. Provisions for granted leave from the university is found within the form.  Please contact the CSULB Veteran Services office for more information.    Military Leave Form (PDF)
Petition for Exception to Academic Policy  This form is commonly used to petition for the following items: Exception to Undergraduate Limit Repetition of Courses (28 units)*, Exception to Undergraduate Limit Course Withdrawal (18 units)*, Academic Renewal*, General Education Course Substitution/Waiver and Graduation Requirements. It is recommended to attach documents along with this form so that the committee fully understands the reasoning behind the petition.  *Some petitions may require a non-refundable $10 late processing fee. Petition for Exception to Maximum Unit Load

Please use this form to petition to exceed the maximum unit load for the Fall, Spring, Summer, or Winter terms.   An exception to the unit limit will be granted only if there is both demonstrated need for the overload (usually to avoid undue delay in graduation or required by major department) and demonstrated ability to succeed in a heavy load. A minimum 3.0 GPA is usually required.

Terms:

  • Fall/Spring - Petition to exceed the maximum unit load of 18 units.
  • Summer - Petition to exceed the maximum unit load of 7 units in a six-week session or 10 units in overlapping sessions totaling 9 weeks.
  • Winter - Petition to exceed the maximum unit load of 4 units.

Reinstatement Petition Please use this form if you are a disqualified student, who has been out of attendance for at least one fall or one spring semester.  To be considered for reinstatement, a disqualified student must meet the requirements for reinstatement.  Please see Academic Standing and Appeals for more information.   Reinstatement Petition Form (PDF)

This form should only be used in the following circumstances:

- The deadline has passed for your graduation term.

Request to Change Graduation Term  This form is used to change your graduation term.  You may or may not be required to submit a Timely Graduation Appeal with the request depending on your individual circumstance. Request to Change Graduation Term (PDF)
Timely Graduation Appeal Guidelines  This form is used for students who must delay graduation and/or are requesting a change in academic objectives after establishing a graduation term must submit an appeal as well as the appropriate Application for Graduation-Special Considerations Only or Request to Change Graduation Term form. Undergraduate Request for Reactivation of Graduation This form is for students who previously filed for graduation, didn't maintain continuous attendance, and believe all degree requirements have been met. Continuous attendance is defined as attendance in a course at a California Community College, CSU, or UC campus for at least one unit of credit in at least one semester each calendar year. CSULB extension courses DO NOT count toward continuous attendance. Students may be required to resubmit transcripts from all other colleges/universities attended. Students who have been disqualified from the University and have not yet been reinstated are not eligible for reactivation.
