How to Proceed
- To begin the process of completing your Immunization Requirement, click on the link to the Patient Portal.
- It is required that you submit your records in English. If we cannot interpret your records, you may need to take further steps, such as getting laboratory tests to show proof of immunity (titers) or you may need to receive immunizations at the CSULB Student Health Services. Please note that titers will only be accepted for MMR, Varicella, and Hepatitis B (if applicable). If you have both an immunization and titer, please upload the immunization record only, not both the immunization and the titer.
