FERPA

You may be accompanied by a support person or advisor during your student conduct meeting(s). Please note that the role of a support person or advisor is to provide support and guidance throughout the process. They may consult with you during the meeting but may not speak on your behalf, represent you, or otherwise participate directly in the conference.


  1. Click the FERPA Authorization Form link.
  2. Enter your student information (name and email address) and indicate where you would like a copy of the completed form sent.
  3. Complete all required fields in the DocuSign form.
  4. Include the name of the individual you are authorizing to access your educational records and attend your student conduct conference.
  5. Select the date through which you would like the FERPA Authorization to remain valid. Please choose a date that allows your support person/advisor to participate throughout the student conduct process, including any follow-up meetings that may be necessary.
  6. Review the form for accuracy and electronically sign where indicated.
  7. Submit the completed form through DocuSign.
  8. Ensure the form is submitted at least five (5) working days before your scheduled conference.

Pursuant to the CSU Student Conduct Process, if your advisor is an attorney, you must provide our office with the attorney's name, address, and telephone number prior to the conference.
