An Announcement on Vaccine Certification

Dear student,

This is a reminder to complete your Student Vaccine Certification online by August 23rd. 

If you've received the first dose of a vaccine that requires two doses, but not the second dose, you won't be able to submit the form yet. As soon as you receive the second dose and your vaccination card is updated, please submit the form.

Even if you're enrolled online in online classes and will not be accessing CSU facilities or participating in any CSU activities, programs, or instruction in person, your certification is still required. You can indicate that you will not be participating in any in-person activities or classes when you submit your form.

Dear Beach Students:

The university is now requiring all students to submit online COVID-19 vaccination certifications prior to the fall semester.


STEP 1:


Student Vaccine Certification service tile in the Campus Single Sign-On (SSO) system.By August 23rd, visit the Campus Single Sign-On (SSO) webpage and complete the Vaccine Certification web form.  

Refer to the tutorial video for guidance on submitting this web form. You will be asked to upload documentation of your vaccination, which in most cases will be a PDF or JPG of your completed immunization card. We recommend using a smartphone to obtain this image. This website meets appropriate standards to process, store and transmit personal health information. If necessary, the web form includes functionality to claim a legitimate medical or religious exemption.

If you are unable to attest to having been fully immunized for any reason (including exemptions) by August 23rd, you will be notified about your required participation in a weekly COVID-19 surveillance testing program.


STEP 2:


COVID-19 All Student Prescreen service tile in the Campus Single Sign-On (SSO) system.Complete the COVID-19 pre-screen in the Campus Single Sign-On (SSO) webpage every time you visit campus. 

Various locations on campus will require you to present your green check mark indicating you are safe to enter the campus.

Students who have not completed the online form by September 30th will have an academic hold placed on their record.
