F. MAKE-UP POLICY:

Each student must take all lecture exams. One lecture exam may be made up if missed for a serious reason such as illness or accident. In order to be eligible for making up a missed lecture examination, you must promptly submit (within one week) written documentation supporting the cause for your legitimate absence. It is your responsibility to contact the instructor and provide the necessary documentation. The make-up exam must be taken before the next regularly scheduled 
examination.


G. WITHDRAWAL POLICY:

A copy of the School of Natural Sciences withdrawal policy is available from the Department Office. Please read it carefully as we will adhere to it strictly. Some critical deadlines to which you should pay particular attention to are listed below.:

H. WITHDRAWAL POLICY: 

The School of Natural Sciences withdrawal policy is reproduced below as it is different from the University withdrawl policy.  The main difference is that the withdrawal deadlines are earlier.  A hard copy is available from the Department Office.  Please read it carefully as we will adhere to it strictly. Some critical deadlines to which you should pay particular attention to are listed below: 


1/24 – 2/6/11                    I.     WITHDRAWALS DURING THE FIRST TWO WEEKS OF INSTRUCTION.  Students may withdraw during this period through MyCSULB (until 2/6/11) and the course will not appear on their permanent records.  After this period, a “W” will appear on the permanent record of the student.  

2/7 – 4/22/11    II.       WITHDRAWALS AFTER THE FIRST TWO WEEKS OF INSTRUCTION AND PRIOR TO THE FINAL THREE WEEKS OF INSTRUCTION. 

A.        Instructors shall provide the students with an opportunity for demonstration of competence, relevant to the determination of their final grade in the course, as early as is reasonable and no later than the end of the seventh week of instruction.

B.         The request and approvals shall state the reasons for the withdrawal. 

C.        During this period of time, withdrawals are permissible only for serious and compelling reasons.   Students should be aware that the definition of “serious and compelling reasons” as applied by faculty and administrators may become narrower as the semester progresses. Written approval must be obtained from both the Instructor and Department Chair

 4/23-5/13/11    III.      WITHDRAWALS DURING THE FINAL THREE WEEKS OF INSTRUCTION.

                                                               Withdrawals during the final three weeks of instruction are not permitted except in cases such as accident or serious illness where the circumstances causing the withdrawal are clearly beyond the student's control and the assignment of an Incomplete is not practical.  Ordinarily, withdrawals in this category will involve total withdrawal from the campus except that a Credit/No Credit grade or an Incomplete may be assigned for courses in which sufficient work has been completed to permit an evaluation to be made.  Grade standing in the course will not be considered, provided the above criteria are completely satisfied.  Requests to withdraw must be accompanied by supporting documentation.  Written approval must be obtained from the Instructor, the Department Chair, and the Dean*.

*A Special form (see reverse side of this form or the college or department offices) for withdrawal during the last three weeks of instruction must be attached to the University’s “Request to Withdraw from a Class” form prior to seeking the Dean’s signature.

Always file a drop form if you do not intend to continue in any course. It is good insurance against getting a "U" in the course. Also, it is requested that you inform your instructor if you drop the course early in the semester (first two weeks); this will give us the opportunity of giving your space to someone else.

Withdrawals from this course will only be allowed in accordance with University and College policies. Please be aware of the more specific and restrictive withdrawal policy for the College of Natural Sciences and Mathematics. Withdrawals during the first two weeks of instruction will not appear on the student's permanent record.

Withdrawals after the first two weeks and prior to the ninth week of instruction are permissible only for serious and compelling reasons. Academic progress unsatisfactory to the student is considered a serious and compelling reason during this period. Instructor and department chairman signatures on drop form are required.

Withdrawals following the eighth week but prior to the final three weeks of instruction are permissible for serious and compelling reasons, but during this period, unsatisfactory academic progress is not considered a serious and compelling basis to drop a course. Circumstances must be shown that preclude the student from attending class or from any effective opportunities to study. In addition to the normal withdrawal form, a special form must be completed, and instructor and department chair signatures are required.

Withdrawals during the last three weeks of instruction are permissible only for serious accident or illness and involve a total withdrawal from the University. Detailed written documentation must accompany withdrawal forms. Instructor, chair, and college dean signatures are required.


H. CHEATING/PLAGIARISM:

Cheating and plagiarism are in violation of the California Administrative Code, Title 5, Section 41301. CSULB has adopted a specific policy with respect to the violations of this nature (see the Bulletin or Schedule of Classes). Any student in violation of this code and policy in any assignment or examination related to this course shall be subject to the options specified in the policy statement. This may result in the student receiving a failing grade in the course or, in certain circumstances, being expelled from the University.
