REQUEST FOR NOMINATIONS FOR 2013 COMMUNITY SERVICE AWARD

Nominations are being requested for the 2013 Community Service Award. There are four awards per year — one each to a CSULB student, faculty, staff, and alumnus/a.

12 noon, February 18, 2013

Description and Purpose:

These awards are designed to recognize and encourage service by members of the CSULB family to the community within which they live. The awards are sponsored by the Academic Senate, the Staff Council, the Alumni Association, and the Associated Students.

Criteria

A wide range of service activities are eligible, including political, religious, and social activism. In all cases, sustained involvement and especially significant results will be considered. Special attention will be given to noteworthy accomplishments within the past 12 months. Preference will be given to unpaid volunteer service which is not part of one’s normal and expected occupational duties.

Eligibility

CSULB faculty, staff, students, and alumni (to include all former students and friends of the University). Self-nominations are encouraged. Awardees may reapply five years after receiving an award.

For a nomination to be considered, please submit:

  1. Completed Nomination Form
  2. Narrative Summary. The narrative should be a maximum of 500 words. It should include the following information.
    • Description of the service activities (including dates and nature of service, unless provided in a resume or CV)
    • Significant accomplishments or outcomes, both overall and in the last 12 months
    • Value of the service to the community served
    • Any benefits to CSULB as a result of the nominee’s involvement
    • Whether the activities described were paid or voluntary
    • Any other factors which the selection committee should consider
  3. Resumé or Curriculum Vitae (optional) including a list of service activities
  4. Letters of Recommendation (one required, up to three accepted). Letters may be submitted with the packet, or may be sent in separately to the individuals listed below, but must be submitted by the deadline.
  5. Nominations will be judged by a committee selected by the sponsoring group; deliberations of the committees are confidential.
