NOMINEE SUBMISSION FORM
Deadline to submit nominations: 12 noon, Friday, March 3, 2006
Please review the attached cover sheet prior to completing this form. All submissions must be by U.S. Mail or hand carried to appropriate location.
Nomination is for a: (choose one)

- CSULB Staff Member
- CSULB Alumnus/a
- CSULB Student
The following additional materials must be submitted with the packet:
- Narrative Summary. The narrative should be a maximum of 500 words. It should include the following information.
Description of the service activities (including dates and nature of service, unless provided in a resume or CV)
- Significant accomplishments or outcomes, both overall and in the last 12 months
- Value of the service to the community served
- Any benefits to CSULB as a result of the nominee’s involvement
- Whether the activities described were paid or voluntary
- Any other factors which the selection committee should consider
- Resume or CV (optional) including a list of service activities
- Letters of recommendation (one required, up to 3 accepted). Letters may be submitted with the packet,
or may be sent in separately to the individuals listed below, but must be submitted by the
March 3 deadline.
