Media Reservation Form

CSULB Faculty may use this form to have CSULB owned video cassettes or DVDs reserved for classroom use for a particular date. Videos that are not in the CSULB collection may be ordered through ILLiad Interlibrary Services . Since there is no form in ILLiad specific to media items, please use the form provided for "books" to make your media requests. Note that not all media items will be available for loan from other institutions. The Reserve/Media/InterLibrary Services Desk will do its best to fill your request and will provide you with status of your request.

This form can accommodate requests for up to 5 items. If you have more than 5 items, after submitting the form, click on "Return to Media Reservation Form" to return to the blank form and enter the information for your next item(s).
