Family PACT

Family PACT is a government program that provides clinical services for family planning and reproductive health at no cost to eligible California residents. All Family PACT information is confidential and program enrollment is completely voluntary. Students are free to withdraw from the program at any time or to refuse any service. After enrolling in Family PACT, students will receive a teal HAP card which can be used at any provider that offers Family PACT in California.

Health services received through Family PACT must be clinically indicated in order to be free. Family PACT offers birth control methods, morning after pill, urine pregnancy test, STI testing and treatment, and annual exams.

How to Enroll or Re-Certify in Family PACT

Please follow the steps below to check your eligibility for Family PACT and to complete the Family PACT application.

First download and print out the Family PACT Application (CEC form). To finish your enrollment, you will need to complete your form and bring it to the SHS to meet briefly with the SHS Enroller. Make an appointment with the Family PACT enroller or let a staff member know you would like to enroll.

If you are already enrolled, re-certify your Family PACT card before it expires. Your card is active for one year from date of enrollment. To speed up re-certifcation process in the SHS please read the instructions above, complete your CEC Form and then bring the completed form to the SHS Family PACT enroller. Please remember to bring your completed CEC Form.

Use of Outside Pharmacy

If you are leaving the area for any reason make sure you have enough prescription supplies of your preferred contraceptive method. Remember that Family PACT is only good in California. To locate a Family PACT provider in your area, call 1-800-942-1054.

Family PACT Client's Use of Outside Pharmacies

Wherever you are in California, you can go to any retail pharmacy (eg. Walgreens, CVS or Costco) to get a Family PACT prescription filled. Some pharmacies have had problems with Family PACT prescriptions so the SHS suggests that you present the Family PACT Outside Pharmacy Letter whenever you need to fill a prescription at a pharmacy other than the SHS pharmacy. To refill a Family PACT prescription at a California retail pharmacy you can present your prescription container with its label, and the pharmacist can call the SHS Pharmacy at (562) 985-1561 to have your prescription transferred.
