CSULB Federal Disclosure Instructions

This is the form for reporting economic interests when funds are received from federal governmental sources requiring such disclosures, currently only the National Institutes of Health and the National Science Foundation. In accordance with NSF and NIH guidelines, it is the responsibility of the PI to update their financial disclosure if a new reportable "Significant Financial Interest" is obtained during the term of the project.

Exemptions

Exempted from disclosure are:

  • your regular salary, royalties or other remuneration from the University (or auxiliaries); salary, royalties or other payments from other sources that, when aggregated for yourself, your spouse, and your dependent children over the next twelve months are not expected to exceed $10,000; income from seminars, lectures, or teaching engagements sponsored by public or non-profit entities;
  • income from service on advisory committees or review panels of public or non-profit entities; an equity interest ... (when applying to federal programs, or foreign, state, or local programs involving U.S. federal funds) that when aggregated for yourself and your spouse and dependent children does not exceed $10,000 in value, as determined through reference to public price or other reasonable measures of fair market value or less than a 5% ownership interest in any single entity.

Reportable Information

Therefore, conditioned by the definitions and exceptions above, disclose for yourself, your spouse and your dependent children all of your financial interests, exceeding the limits stated above, that would reasonably appear to be affected if the project were funded.

In determining whether to disclose an interest, please understand that the CSULB Independent Review Committee is charged with responsibility for determining whether or not any particular interest constitutes a potential conflict. Thus, you should mention any interest about which you are uncertain.
