You may also email it as an attachment by creating the report as above or downloading the report form below and creating your report. If you e-mail your report to the Office of University Research as an attachment without using the above link, please send to jyoung6@csulb.edu with cc: jtill@csulb.edu and djorgens@csulb.edu. In both cases you should cc: your Department Chair and College Dean.
You may also send your report as a hard copy via campus or US mail with copies to your Chair and Dean.
