The basic protocol application form was adapted from that used at Cedars-Sinai Medical Center. It is designed to be downloaded from the IACUC website, completed on a word-processing system, and transmitted electronically to the Attending Veterinarian for preliminary review. The researcher should make all changes to the draft application before submitting it electronically to the Office of University Research for further distribution to members of the IACUC.
The application is a format which expands to meet the needs of each project. It also serves the researcher as the baseline document from which subsequent modification or annual reports are assembled.
