Directions:

 

  1. Click the ”Staff Information Form” link below
  2. Save the file directly onto your computer.
  3. Complete the form then save it.
  4. Send the form as an email attachment to mthomas@csulb.edu

Staff Information Sheet

 

Supplemental Instruction Reports:

Listed below are two Excel spreadsheets. Please take the following steps while using these sheets:

1.   Save the file directly onto your computer.

  • Right-click (or Ctrl+click) the link
  • Select Save Target As... or Save link As...
  • Save it to the desired directory on your computer or to disk.

2.   Move to each cell by hitting the Tab key.  To move back a cell, hold down 

       Shift and hit the Tab key.

3.   Save your work often.

4.   Before printing, reduce the worksheet to fit the page.

  • Click on the worksheet
  • Go to File > Page Set up...
  • Go to the Page Tab
  • In the Orientation box, select Landscape (For Grade/Attendance Report only )
  • In the Scaling Box, select Fit to 1 page wide by 1 tall

5.   Before printing, center the spreadsheet on the page.

  • Click on the worksheet
  • Go to File > Page Set up...
  • Go to the Margins Tab
  • Check Horizontally and Vertically
