Travel forms & information (University & Foundation):

  • Travel Request Form (online form that is converted to pdf at the end). Important Note: the department chair has to sign underneath the traveler's signature; ask Irene Howard for the departrment reference number; Only insert "Dr." under "Appropriate Administrator". (dated 10/03/2011)
  • Travel Expense Estimate (excel form; dated 07/2010)
  • Travel Expense Claim (excel form; dated 04/2011). For faculty. See foundation forms below for students.

The above forms can also be found on CNSM form & procedure website or on the controller's office form website.

  • International travels: "travel authorizations especially to countries on the state
    government travel alert and warning list must be received in the Office
    of the Provost no later than 30 days prior to travel date.
    Please refer to the "Due Date" section of the Academic Affairs Travel
    Guidelines
    . The lead time is needed to ensure the international insurance may be
    bonded following approvals by the Offices of the Provost, President and

  • CSULB Foundation travel forms.
  • Student travel: students always need to fill out the attached Foundation travel form. (the attached form is a word document; excel and pdf formats are found under "Travel Forms" on the form website of the Foundation).
