Absence from class form:

• Faculty absent from or cancelling class must fill out the appropriate "Notice of Absence from Class/Class Cancellation" (word doc) form.

Picture release form for the website:

• The CSULB image release form and instructions must be signed by any person identifiable on a picture that is uploaded on the website related to the University.
