COTA Truck Checkout Procedure

How to reserve the COTA truck:                         

 Check truck’s availability online at http://sites.google.com/site/cotatruck/

If the date and time is available contact Kevin O’Grady at 5-4366 or kogrady@csulb.edu to schedule the reservation.

Where to pick up the COTA truck keys:

 The keys to the truck are located in the Dean’s Office (AS-115) at reception desk (office hours are 8am-5pm, M-F).

The driver will need to sign the truck out indicating date, driver’s name, destination, and what activity or event truck will be used for.

Where to pick up the COTA truck:

 The truck is parked at the theatre loading dock or in a space in lot 6 in close proximity to the loading dock.

How to fuel up COTA truck:

 If the fuel level in the truck is less than 1/4 tank when returning, it should be refilled with unleaded gas at the Maintenance Yard.  This requires a key card which is kept in the truck. If absolutely necessary, (for example, when travel distance exceeds gas tank capacity) the truck can be filled up at a gas station using petty cash.  Always note the amount of gas pumped in the comments section of the Usage Log.

COTA truck rules and regulations:

 *NOTE: Please see the COTA Truck Policy Document attached for complete truck rules & regulations.

Maximum occupancy of the truck is two people.  Driver and passenger must wear seat belts.  At no time is any person(s) allowed to ride in the bed of the truck, no matter how short the trip distance.

 NO SMOKING is permitted in the truck.

 The truck should be left clean and doors must be locked after usage.  It is the driver’s responsibility to make sure this happens.

Eligible COTA truck drivers MUST:

Be a CSU faculty, staff or student assistant who is currently in a state-funded pay status position.

  • Possess a valid California Driver’s License.
  • Have satisfactorily completed the approved CSU defensive driving course.
  • Have read and signed a copy of the campus ‘Driving Risks and Responsibilities’ document.

All drivers of the vehicle will be responsible for the driving practices and principals listed in the COTA Truck Policy document, as well as those taught at the defensive driving course.
