If you are a distance education student, please contact your SDS for application materials and deadlines. All other students may apply now using the online CSU-Mentor Graduate Application Form or a mail-in admissions application booklet. CSU-Mentor is the California State University's online resource to help you quickly and easily apply for admission on the Internet. If you do not have access to the Internet, you can request that an admissions application booklet be mailed to you by calling our automated application line at (562) 985-1655 or (562) 985-4145. Ask for the Undergraduate Application for Admission. If you use the mail-in application form, the code number you will need to write-in for the Department of Professional Studies is #08395. Please make sure you meet application deadlines. Note the following as you complete your online or paper application:
Arrange for your transcripts to be sent to CSU Long Beach. You should arrange to have official transcripts sent at the same time you submit your application. If you have college transcripts at home, they should remain in the college's official, sealed envelope. Send (or arrange for your transcripts to be sent) to the following address:
Office of Admissions and Records
California State University, Long Beach
1250 Bellflower Blvd.

Note: We recommend that you send a photocopy of your entire admissions packet (including transcripts if you have them) to the Department of Professional Studies. We will start a file for you while the University processes your application. Send your photocopied application to the following address:
