49er Shops is proud to provide outstanding benefits to all full time employees. You will find that they are comparable to the benefits offered by the University and add a significant amount to the total compensation package offered upon employment.

The following is a summary of what we offer. If you have questions or want a more in depth description of the plans, you may contact 49er Shops Human Resources at (562) 985-7854

Benefits become effective the first of the month after 30 days of employment. All full time employees, working 30 hours or more per week, are eligible for all benefits

PMI is an affiliate of Delta Dental Plan of California and administers the DeltaCare program. DeltaCare is a dental HMO covering over 650,000 Californians through a network of private practice dental offices, which is composed of established dental practices - not clinics. As an enrollee in the DeltaCare program you select one office from any of these dental offices for your and your family's needs.

With this program you MUST use only those dentists on the list. If you use a dentist who is not on the list, you will NOT be covered. You and your family members will receive benefits in the same provider office.

You may CHANGE A PANEL DENTIST you have selected to another by phoning or writing DeltaCare by the 21st day of the month. The Customer Relations Department will assist you in the transfer, which will take effect the first day of the following month.

PMI also maintains A PANEL OF SPECIALISTS. Should you require specialty services, PMI will refer you to a specialist in your geographical area, upon prior authorization.

THREE TO FOUR WEEKS is a reasonable amount of time to wait for a standard appointment, if you can accept the first available appointment.

NO CLAIM FORMS
The dental office you choose provides all primary dental services. There are no claim forms to complete nor percentage of usual charges for you to pay.

NO DEDUCTIBLES
There are no required deductibles to pay, so your benefits begin immediately.

NO DOLLAR LIMIT OF DENTAL BENEFITS
No annual maximum.

PREPAID PLAN SAVES ON DENTAL COSTS
Your out-of-pocket savings are substantial. You know the exact cost prior to treatment and this aids in better fiscal planning for you and your family.

NO PRE-EXISTING CONDITIONS RESTRICTED
These conditions are not excluded. Exception: Work in progress - preparation for crowns, root canals, impressions for dentures, etc.

SPECIALTY SERVICES
The DeltaCare program offers services in dental specialty areas. These include periodontics (treatment of diseased gums and bone), endodontics (root canal therapy), and oral surgery procedures.

You are also covered for OUT-OF-AREA DENTAL EMERGENCIES. This program will pay dental expenses incurred up to a maximum of $50.00 during each 12 calendar months. "Out-Of-Area" means 35 miles or more from your PMI participating dentist's office.

HOW IT WORKS
When you enroll in DeltaCare, select a panel dental office from the list. After you have enrolled, you will receive a DeltaCare membership card and an Evidence of Coverage that fully describes the benefits of your dental plan. The card will have the address and telephone number of your panel dentist. To receive all necessary dental care covered by the plan, simply call your selected panel dentist to make an appointment. Dental services which are not performed by your panel dentist or not authorized prior to treatment by PMI will not be covered by the DeltaCare program.

WHO CAN JOIN
If you are a full-time employee (30 hors per week or more), you can enroll in DeltaCare. You can also enroll your family members - your lawful spouse and unmarried children; including step-children, legally adopted and foster children up to the age of 24.

The DeltaCare program provides all reasonable and customary dental care, if care is rendered by your PMI panel dentist. There is no cost for covered services, except for CO-payments on certain procedures. You can refer to the Human Resources office to get information about specific limitations and exclusions, which are included in the plan. You may also obtain a list of copayments, associated with certain procedures.

The PMI Customer Relations Department will assist you in all matters pertaining to the DeltaCare program. You may reach a PMI representative at (800) 422-4234. Office hours are 8:00 - 4:30 Monday through Friday.


Ameritas LogoAny full-time active employee working at least 30 hours per week is eligible for Personal Insurance.

ELIGIBLE DEPENDENTS are: a lawful spouse, unmarried children, including adopted children and children covered under a qualified medical child support order, up to the age of 25 or over that age if they are mentally or physically handicapped.

If a full-time active employee has eligible dependents, then he is eligible for dependent insurance.

Retirees have the same level of coverage as active employees.

The employee may use a participating provider or a non-participating provider.

There is a freedom of choice of any provider at all times. The deductible is less, if the employee decides to use a participating provider.

When you use a participating provider, a negotiated fee schedule is used which is intended to reduce your out-of-pocket costs.

THE BENEFIT PERIOD is the period from January 1 of any year through December 31 of the same year. But during the first year a person is insured, the benefit period is the period from his or her effective date through December 31 of that year.

DEDUCTIBLE AMOUNT:
Type I (A) Procedures - once per lifetime - $0
Combined Type I (B) - once per lifetime - $50
Type II Procedures - each benefit period - $50
Maximum Family Deductible - each benefit period - $100

COINSURANCE PERCENTAGE:  
Type I (A) Procedures 100%
Type I (B) Procedures 80%
Type II Procedures 50%

Maximum amount for each benefit period is $2000.

You may refer to the Human Resources office in order to obtain a table of different types of dental procedures and a list of participating dentists. General provisions of the contract including limitations and exclusions are available as well.

No benefits are payable for a procedure that is not listed.

ITT Hartford's employee benefit programs include: Managed Disability, Long Term Disability, Short Term Disability, Life and Accidental Death and Dismemberment.

SHORT TERM DISABILITY
This plan provides you with short term income protection if you become disabled from a covered accident, sickness, or pregnancy.

The MAXIMUM DURATION OF BENEFITS for a Disability is 13 weeks.

The WEEKLY BENEFIT MAXIMUM: 66.66% of your weekly earnings; or $1500, reduced by other income benefits.

LONG TERM DISABILITY
Benefit percentage: 66.66% of monthly income loss.

MAXIMUM MONTHLY BENEFIT: $6000

MINIMUM MONTHLY BENEFIT: $100

You must be disabled for 90 consecutive days,before benefits become payable.

Prior to age 63 your benefits are payable to normal retirement age (as stated in the Social Security Act) or 48 months, if greater.

LIFE
If you should die as an employee here, your beneficiary will receive the equivalent of your annual basic earnings.

If you reach age 65, your amount of life insurance and maximum benefit (for AD&D) will be subject to reductions.

Your beneficiary designation(s) must be in writing and on file with the Human Resources office.

AD&D
If you have an accident as an employee here, and you lose your life, your beneficiary will receive two times the equivalent of your annual basic earnings

The Hartford will pay a benefit if you suffer accidental bodily injury and:

  1. a loss results directly from such injury; and
  2. such a loss occurs within 90 days after the accident causing the injury.

For the loss of a hand, foot, or eye, you receive one-half the maximum benefit.

For more than one of the above, resulting from one accident, you receive the maximum benefit.

General provisions of the contract, limitations and exclusions are available in the Human Resources office.

Vacation - Holidays

 

Annual paid vacation is granted to every employee. The Forty-Niner Shops encourage all employees to take at least one half of the vacation time that they accrue each year.

The maximum accumulation of vacation hours permitted to be carried over to the next year is 384 hours (48 days).

Employees become eligible for vacation after one pay period Scheduling of vacations will be approved by the department manager.

Legal holidays for all administrative and staff employees are the following:

- New Year's Day
- Martin Luther King's Birthday
- President's Day
- Lincoln's Birthday
- Cesar Chavez
- Memorial Day
- Independence Day
- Labor Day
- Admissions Day
- Columbus Day
- Veteran's Day
- Thanksgiving Day
- Christmas Day
- Each employee is entitled on a Personal Holiday.

Any Holiday which falls on a Saturday shall be observed on the proceeding Friday. Any Holiday Which falls on a Sunday shall be observed on the following Monday.


Sick Leave Accumulation

Sick leave is accumulated at the rate of one average working day for each completed month of service. There is no limit on the amount of sick leave that an employee can accumulate. Sick leave may be used only for absences due to illness or illness in the immediate family. All sick leave requests must be approved by the division manager


OPTIONAL BENEFITS

The Forty-niner Shops has established THE EDUCATIONAL POLICY to assist staff members in obtaining their career development goals. The policy gives full-time staff members the opportunity to attend classes at reduced rates. Employees who do not qualify for this policy are student assistants and part-time or temporary employees.

The two options available in this program are work- related courses and career development courses. The books required for course work are available at the University Bookstore and may be checked out for the semester through the General Manager's office. Used books will be issued if available. New books may be checked out only when used books are not available.

Career development courses are those required for a Bachelors or Masters Degree Program. While job promotions are not guaranteed as a result of completing courses in the career development plan, an employee may be better prepared and more competitive for job vacancies as a result of having completed specific courses or a degree program. Classes for career development must be taken for academic credit.

Work-related courses directly improve skill or enhance knowledge for performing duties in an employee's current position. Enrollment may be voluntary or at the direction of the employee's supervisor or manager.

The following procedures must be completed prior to registration:

    • A Career Development Plan form must be submitted to the division manager
    • A Tuition Reimbursement Request must be approved by the division manager and forwarded to the General Manager
    • There is a limit of two courses that can be taken each semester
    • At the close of each semester a Course Completion Card must be signed by the instructor and returned to the General Manager's office
    • The General Manager will authorize tuition reimbursement after receiving
  • Course Completion Card indicating a grade of C or above
  • Verification of paid tuition
  • Borrowed textbooks

All the forms mentioned above may be obtained in the General Manager's office.

The amount of tuition reimbursement is:

  1. Up to six units per semester taken at a community college shall be paid for in full for undergraduates (estimated amount $60.00)
  2. Up to six units per semester/quarter taken at another University shall be 70% reimbursed.
  3. Up to six units per semester taken at CSULB shall be 80% reimbursed for eligible employees.

 

ANOTHER OPTIONAL BENEFIT is available to those employees who are graduating from CSULB. The University Bookstore is offering them free cap and gown rentals.

EVERY EMPLOYEE IS ELIGIBLE TO JOIN THE CREDIT UNION. You may obtain a brochure from the Human Resources office that describes different services available and provides the office hours, locations and phone numbers. One of the advantages to membership is convenient payroll deductions to savings or loan accounts.

ALL FULL-TIME BENEFITED EMPLOYEES ARE OFFERED A SUBSIDIZED MEMBERSHIP IN FROGS WORKOUT FACILITY FOR ONLY $7 PER MONTH. Just go to Human Resoruces to complete a payroll deduction forn.

University Bookstore DISCOUNTS
1) All employees receive a 20% reduction on certain general merchandise in the University Bookstore.
2) All University Bookstore employees and Dining Services full-time employees receive a 15% discount on textbooks.
3) Dining Services (full-time and part-time) employees receive a discount on meals.

There are some exceptions to these policies, so consult your supervisor for more specific information.

Employee purchases may be made at any register in the University Bookstore.

ITEMS OF INTEREST AND SPECIAL ANNOUNCEMENTS will be distributed regularly along with paychecks. Other notices of programs and events are usually posted on employee bulletin boards.

Each year the Board of Directors and General Manager host SPECIAL STAFF EVENTS including a holiday party. Please plan to attend these events to get to know your fellow employees

CHECK CASHING PRIVILEGES are extended to all employees. Employees may cash personal checks up to $50 or payroll checks issued by the 49er Shops. All checks may be cashed at the Customer Service counter in the University Bookstore. A current employee I.D. is required in order to cash a check.

The EMPLOYEE ASSISTANCE PROGRAM offers professional consultation to assist 49er Shops employees with solving work-related problems, as well as any personal concerns. These may include problems related to stress, family, finances, legal issues, emotional distress, alcohol and drugs, or health.

Your contact with EAP is free of cost. Discussions of any problems will be handled with strict confidence. Records are maintained solely for EAP use. No information will be placed in personnel files.

THE LONG-TERM CARE PROGRAM is an option for all salary employees in our corporation. The program provides supportive medical and personal assistance to people with chronic illnesses or disabilities. LONG-TERM CARE PROGRAM is managed by the California Public Employees' Retirement System. LONG-TERM CARE services can be provided at home, through community-based settings such as adult day care centers, or in assisted living and nursing care facilities.

49ER SHOPS SPONSORED TAX SHELTER ANNUITY RETIREMENT PROGRAM

*All hourly employees have the option to participate in this retirement plan.

*In order to participate in the plan you must be at least 18 years of age and have completed 1,000 hours of service.

*You may contribute a maximum of $12,000 a year. Your maximum may be more under certain circumstances. You may contact the Human Resources office for more information.

-You may change the amount of your contribution once every 90 days.
-You can stop at any time.

*FIXED AND VARIABLE are two investment plan choices. You may contribute to one or both at the same time as long as the minimum amounts of $10.00 per month for FIXED and $50.00 per month for VARIABLE are satisfied.

-FIDELITY & GUARANTY LIFE is the FIXED investment choice for your plan. All moneys are guaranteed by the company and earn a rate of return that is guaranteed never to be less than 4%. You receive a 5% bonus on each contribution and you may borrow from your account. Ask your Human Resources office for details.

-GREAT AMERICAN RESERVE is the VARIABLE investment choice for your plan. All moneys are not guaranteed and will fluctuate in value. You may transfer moneys among the available funds in the plan anytime at no charge.

*THERE ARE NO FRONT END SALES CHARGES FOR EITHER OPTION. There are back end charges that may apply. Please contact Human Resources office for details.

There is no annual charge for the fixed option. There is a $30.00 annual charge for the variable option.

A 10% tax penalty will apply for hardship and termination withdrawals.

 

PUBLIC EMPLOYEES RETIREMENT SYSTEM

*ALL SALARY EMPLOYEES are automatically enrolled in the Public Employees Retirement System (PERS).

*The State Employees' Retirement System, the predecessor to the Public Employees' Retirement System of the State of California (CalPERS), was created after voters approved a constitutional amendment authorizing legislation to establish a pension system for State employees. This legislation became effective January 1, 1932.

*The Public Employees' Retirement Fund (PERF) is a defined benefit retirement plan which provides benefits based on members' duration of service, age, final compensation, and benefit formula. In addition, benefits are provided for disability, death, and survivors of eligible members or beneficiaries.

*Members of the PERF become fully vested in their retirement benefits earned to date after five years of credited service.

*The benefits are funded by contributions from members and employers, and earnings from investments. Member and employer contributions for the PERF are defined by law and depend on the respective employer's benefit formulas.

Employees eligible for PERS retirement may choose health insurance from a list of carriers who have contracted with PERS. This list may be obtained in the Human Resources office or online at http://www.calpers.ca.gov/default.htm
