Below is a detailed breakdown of all medical, dental, and vision plan options available. To help navigate through Open Enrollment, here are several FAQs for your review.

If you are interested in enrolling into a new plan or adding/deleting dependents, please complete the corresponding plan enrollment form (along with the Open Enrollment Change Notification Form) and return it to Human Resources by Friday, December 12th at 3:00pm.

If you are satisfied with your current medical, dental, or vision coverage and do not want to make any changes, please complete the Open Enrollment Change Notification Form. Your existing coverage selections for 2014 will be rolled over to the 2015 calendar year, at the 2015 plan rates.

For your convenience, here is a summary of all plan rates for 2015.

Medical Plan Option

The Shops will offer Kaiser HMO, Anthem Blue Cross HMO & HMO Select and Anthem Blue Cross PPO.

Your plan options are provided below.

Vision Plans

The Shops will continue to offer the same vision plan benefits as we did last year. Our plan of choice has been VSP.

Additional Programs

Voluntary Life and Accident Death & Dismemberment Plan

This year we will continue to offer a Voluntary Life and Accident Death and Dismemberment Plan (AD&D). This plan will allow you to purchase additional life and/or AD&D coverage for you and your family. They are offered through MetLife, the same carrier who provides your dental, life and disability coverage. Both provide a piece of mind that your family will be provided for should something unexpected happen.

Flexible Spending Account Option

The Flexible Spending Account is a voluntary plan where you elect to have automatic deductions taken from your check and put into a Flex Plan so that you do not pay taxes on that amount. The money that you elect to deduct can either be used to reimburse you for medical expenses and/or for childcare expenses that you incur during the year, depending on the deduction amounts for each plan in which you enroll. Your participation in the Flexible Benefits Plan is voluntary. Enrollment is not automatic, and you must re-enroll each year to participate.

New this year:

The IRS has increased the maximum annual contribution limit for Flexible Spending Accounts to $2550. FSA account-holders can carry over up to $500 into the next plan year, effective for 2014 into 2015. However, after December 31, 2014, the FSA use-it-or-lose-it rule will still apply to amounts over $500. In addition, the $500 does not count toward the $2,550 limit on FSA contributions, which was an Affordable Care Act requirement that started in 2013. That means a total of $3,050 can be contributed to employees´ FSAs for the 2015 plan year, but $500 of that has to be carried over from 2014, and only $500 can be carried into the following plan year.

Benefits Waiver Program

The Forty-Niner Shops offers a Benefits Waiver Program to help manage your benefits coverage. If you have medical, dental, and vision coverage outside of the Shops, the Benefits Waiver Program pays you cash in lieu of medical coverage. You must provide proof of coverage to participate.
