Welcome to our new Open Enrollment Resource Page!

We have created a resource website to help you navigate through all of the medical plan options, medical plan summaries, and monthly premiums available this year.

Open enrollment is your opportunity to review and make changes to your medical, dental and vision plans. The open enrollment period will be held from Friday, November 15, 2013 through Friday, December 6th, 2013. The deadline to submit open enrollment changes to Human Resources is Friday, December 6th at 3:00pm. Elections made during open enrollment will become effective January 1, 2014.

If you decide that you do not want to make any changes, no further action is necessary -your 2013 open enrollment selections will roll over to 2014, at the 2014 plan rates. If you need additional support or computer access to access these online resources, please contact our office and will be happy to schedule an appointment for you.

Employer Contributions

Please note that the Shops has also increased the employer contributions for the 2014 calendar year. These reflect the monthly contributions that the Forty-Niner Shops will make toward your total monthly premiums based on the coverage group you have selected from our plan options.

  • Employee Only — $492/month
  • Employee + One — $1034/month
  • Employee + Family — $1,477/month
